yes they refer to all that the only one they dont refer to is the brachy measurements (cranial index???  sorry too early for my brain)

I think in most cases they try and aim for a 9 or lower as that is the # most typically recommend banding at.  In our case we went from 15, 15 and 17 to 4, 5, and 6 so we were not recommended to do a 2nd band but I can still see it and if I had the time and money I would have tried to convince them to do another so i guess i would say it depends on what they look like, i would want under 6
